@dean i compared this report www.f.waseda.jp with this one www.nisa.meti.go.jp . there are some discrepancies esp regarding #3. we should keep the reports to recognize falsifications later.

15 ton of low-level radioactive water spilled out from a temporary tank because a hose used as a water gauge was detached from the tank wall
(cont) Why hose was detached? Because TEPCO used a piece of duct tape(!) to put the hose on the wall, but the tape fell off.
(cont) TEPCO reported no radiation increase on the ground around the spilled water. (Does this make sense? The water is radioactive.)
